I agree to care for the prop I am hiring and will return it in good condition at the end of the hire period. I will notify the school immediately of any accidental loss or damage.

PARTICIPATION AGREEMENT (REQUIRED FOR ALL STUDENTS) I / My Child will be participating in classes, workshops & performances with Ghawazee Moon Bellydance.

I authorise the school to obtain medical assistance as may be deemed necessary. I agree that I / my child may receive emergency medical treatment, including first aid, and agree to meet any associated medical expenses, including ambulance.

Please note here any medical or other information your teacher should be aware of to ensure your health, safety and enjoyment of dance classes.
